School enrollment

Enter your school's name and the number of male and female students enrolled in each grade.

Important note — school enrollment: This form collects enrollment data for grades 7–12 by sex; however, you should report enrollment based on the grades that make up your high school population.

In most cases, this will include grades 9–12. If your high school consists of grades 10–12, report enrollment for those grades only. If your school includes grades 7–12 within the same building or program, include all applicable grades in your enrollment totals.

If a portion of your student population is generally ineligible to participate in your school's athletic programs, those students may be excluded from the enrollment count. For example, if your school serves grades 9–12 but allows limited participation opportunities for a small number of younger students, you do not need to include full enrollment figures for grades 7–8.

Athletic offerings

Add every sport your school offers, including any sports that are part of a cooperative (co-op) sponsorship. Cooperatively sponsored sports are included in total team counts only when participation meets or exceeds the average team size for your school.

Important note — athletic offerings: When determining which activities to include in your athletic offerings, follow guidance from the Office for Civil Rights (OCR), including the 2008 Dear Colleague Letter. OCR does not provide a single definition of "sport." Instead, it evaluates whether an activity qualifies as a sport based on factors such as structure, administration, team preparation, and competitive opportunities.

Only include activities that meet these criteria and are recognized as part of your school's interscholastic athletic program for the purpose of Title IX compliance.

Do not identify sports as co-ed simply because both males and females are eligible to participate. Assign a sex designation to each sport based on the title of the sport (e.g., Boys Soccer) or the sex that represents the majority of participants (e.g., Wrestling may be designated as Boys Wrestling with female participation). All participants, regardless of sex, must be included in the total count of athletes.

Participation numbers

Enter total participation numbers for each sport. Record both male and female participants for every sport regardless of its sex designation.

Important note — participation data: As of the date of data collection, only participants in sports that meet the criteria to be counted as sports based on OCR's evaluation factors should be included in your proportionality calculation. Be sure that only those qualifying sports are reflected in your final participation counts. You should record your duplicated participation data and only include participants that were on the team as of the first "countable" competition.

For co-op sports, competitive levels will be excluded from the team count if participation falls below the average team size — a note will appear below those sports as you enter numbers.

Competitive levels

Select all competitive levels offered for each sport. Common levels are shown as toggles — click to select or deselect. Add a custom level using the text field below each sport. For sports designated as co-ed, select competitive levels for both the male and female sections, since both males and females may participate on the team even though they compete in a co-ed setting.
