Title IX — Opportunity to Receive Coaching
Fair Field Solutions — Title IX Athletics
Opportunity to Receive Coaching Survey
Complete one form per coaching position.

To self-assess compliance with Title IX Athletics and meet the guidelines set by the Office for Civil Rights (OCR) of the U.S. Department of Education, we are gathering detailed information about coaching duties at our school. This form is designed to collect data related to the availability, qualifications, and compensation of coaches, which are key areas evaluated under OCR's Title IX policy. Your responses will help us assess equity in our interscholastic athletics program by examining how coaching opportunities are distributed, how coaches are assigned, and how compensation is determined. Thank you in advance for your time and participation in this important effort.
